Community Health Assessment & Improvement Plan

Every five years, the Lexington-Fayette County Health Department (LFCHD) in partnership with the Lexington Community Health Improvement Partnership (LEX-CHIP) engages the community in a health assessment and planning process for the purpose of improving health in the Lexington-Fayette County community.

This document has been created to share the process and results of the Community Health Assessment (CHA) and to address the priority issues it identified. The purpose of the Community Health Assessment (CHA) is to learn about the community, the health of the population, contributing factors to health outcomes of identified populations, and community resources available to improve the health of the community. The CHIP takes information from numerous sources, assessments, and the CHA. It creates initiatives using the assets based on the needs of the community to improve the health and well-being of those who live in Lexington-Fayette County.

Goals, objectives, and strategies for each of the priority issues are outlined in this document. The purpose of the plan is to detail how community partners will collectively and collaboratively work together to improve the health and well-being of our community.
